• Book your children into the Swan Hill After School Program

Please note

• Only one After School Hours booking form is required for all your children, unless their term dates vary
• Your childen must be enrolled in the Out of School Hours (OOSH) program before they can be booked into theAfter School program.
• If your child is not enrolled in the OOSH program, click here to fill in an enrolment application first.

Who can use this form

• A parent or guardian who has parental responsibility for the children being booked into the program
• We are not currently accepting after school bookings for St Mary’s or Swan Hill North students. After school options for these students is available by contacting St Mary’s Primary School.

• School details
• Term 1 to 4 booking dates

Please note

• Fees will be charged for Public Holidays from the start of Term 1 if they fall in the term and on the day your child usually attends
• You will be charged for any bookings, even if they are not attended
• Be sure to take into account any known holidays, RDO’s, school camps etc when making your booking
• Parents who do not book could miss out on care or activities on any one day

• We will send you an email receipt to confirm we have received your application
• A staff member will be in contact with you to discuss your booking
• Completing this form is not a confirmation of booking

What happens with your information?

Information requested on this form is required by Swan Hill Rural City Council in order to process your application.

We will handle any personal information you provide on this form in accordance with the Privacy and Data Protection Act 2014. We record this information on our customer databases and make it available to relevant Council staff in line with our Privacy Statement.
