OOSH – Swan Hill Vacation Care Program – Booking Form Swan Hill Vacation Care Program – Booking Form 1Introduction2Family3Attendance4Agreement You can use this form to Book your children into the Swan Hill Vacation Care Program Please note Only one Swan Hill Vacation Care Program booking form is required for all your children, unless their booking dates vary Your childen must be enrolled in the Out of School Hours program before they can be booked into theAfter School program. If your child is not enrolled in the Swan Hill Out of School Hours Care (OSHC) Program, click here to fill in an enrolment application first. Who can use this form A parent or guardian who has parental responsibility for the children being booked into the program While completing this form, we will request Personal details Booking details It will take about 5 minutes to complete this form. Please note You will be charged for any bookings, even if not used, unless the spot is filled from the waiting list. If you are unsure of your needs, book the days you think you might require early so that you don’t miss out. Due to unavailability of staff, numbers are capped for most days. Bookings can be cancelled or altered up until the booking closing date. The cancellation of bookings bookings have closed will incur an absence fee. Child Care Subsidy is only paid for absences that occur once a child has started using a service and only until the last attendance at a service. If your child has not attended Vacation Care or After School Care in the 14 weeks preceding your Vacation Care program booking, any absences at the start or end of the program will not be eligible for Child Care Subsidy. Centrelink will pay the subsidy to you (it will be taken off your account), but will reclaim it 14 weeks later, which will leave you with a debt to our service. Fees must be paid within seven days from the date of invoice. Your invoice is emailed to you each week. Payments can be made at Council offices or by BPAY, your BPAY reference is on your account.
